The PeopleSoft HCM Self Service module allows employees to change their name. You may access this module at the following site: https://selfservice.uth.tmc.edu/hrss/signon.html. Navigate to the Personal Information menu, and select Name Change. You must present your new Social Security Card to Human Resources after submitting your request. You may either present in person at the Human Resources Office: 7000 Fannin, Suite 150, Houston, TX, or scan the social security card and email to the Human Resources mailbox: [email protected]. Name Change requests will not be processed until Human Resources receives a copy of the new Social Security Card. If you scan and email a copy of your card, make sure to white out your social security number except the last four digits. Enter 'Name Change,' your previous name, and emplid in the email subject line. Please note, we can only make a name change with an updated social security card and not a marriage license or a social security card receipt.
