Web-Based Support Program for AD/ADRD Caregivers of Hospitalized Veterans Living with Dementia to Prevent Elder Mistreatment during Care Transitions from Hospital to Home

We propose to evaluate the feasibility and preliminary impact of Hospital GP4C, a web-based support program, when implemented at the key inflection point of transition to home. We will examine effects of the intervention on: (1) key mechanisms related to EM risk; and (2) EM incidence in Veterans with ADRD. This proposal provides a clear path to future NIA behavioral intervention Stage 2-4 projects (e.g., follow-on randomized clinical trials in Veterans and non-Veterans).
