Clinical Informatics and Decision Support
The mission of the Clinical Informatics and Decision Support Core is to innovate solutions leading to personalized, data-driven, and age-friendly care in our community. We seek to leverage information and technology to optimize health care delivery and outcomes for older adults.
Goals of the core
- Unifying clinical care, health information technology, and informatics methods to elevate patient care and support health care decision-making
- Creating systems that support care coordination and incorporate patient preferences
- Developing tools integrated into a learning health system
What we are doing
- Empowering providers and geriatric clinics through clinical informatics efforts, including the creation of new alerts, decision support tools, and queries
- Supporting the development of age-friendly, accessible, patient-centered tools
- Fostering collaboration across departments, schools, and institutions
Future Directions
- Remote monitoring and telemedicine for older patients
- Precision medicine for aging
- Health information exchange for care coordination
- Health behavior monitoring and aging in place
